Antiphospholipid antibody syndrome is an acquired prothrombotic state determined by the presence of antiphospholipid antibodies in patients with venous or arterial thrombosis and increased morbidity during pregnancy. The objective of the work responds to the current interest in AFS-O due to its impact on maternal-fetal morbidity and mortality and its challenge for Primary Health Care. A case is presented of a 26-year-old female patient with a history of obesity, with an obstetric history of her first pregnancy with early neonatal death at 34 weeks due to severe preeclampsia with retroplacental hematoma and severe oligoamnio. In her second pregnancy with dystocic delivery at 32 weeks due to chronic hypertension with added preeclampsia and severe oligoamnio, for which she was moved to Havana City, obtaining a live newborn weighing 1320g and 5/7. In her third pregnancy, Antiphospholipid syndrome was suspected, she was admitted from 12 weeks to the special perinatal care room until delivery, a cesarean section was performed with a gestational age of 39 weeks, resulting in a live newborn weighing 2950g and an Apgar score of 9/ 9. Satisfactory postpartum evolution. The prenatal consultations of this pregnant woman were important since they allowed the identification of risks in order to carry out timely interventions, minimizing these risks, obtaining a good perinatal result.
