An instrument to evaluate the performance of the family medicine resident in a practical exam. Isle of Youth

Abstract

The specialty of comprehensive general medicine was a political decision of the Cuban state, based on the ideas of Commander-in-Chief Fidel Castro Ruz. Assessment is an essential element in all training processes. The study focused on implementing an evaluative instrument as a methodological strategy, specifically an observation guide for the practical promotion exam in the Family Medicine specialty on the Isle of Youth. This tool was developed and applied during the practical exam, enabling actions to be taken to ensure improvements in the quality of learning for residents specializing in Family Medicine on the Isle of Youth. An observational, descriptive, cross-sectional study was conducted from October to November 2023. The study focused on the 23 practical promotion exams of all Family Medicine residents at the Isle of Youth who took the first call for the 2023 academic year. Only 8.69% of residents achieved the maximum score, with the highest score in the 90-99 range being 39.13%. First-year residents had more deficiencies in the practical exam. 64.2% and 57.1% had difficulty with the questioning and physical examination, respectively. All residents passed the promotion exam. Some achieved the maximum score, with scores ranging from 90 to 99. First-year residents had more deficiencies in the practical exams. The sections evaluated, such as the interview, physical examination, and the ASIS defense, were the main deficiencies over the three years, making it necessary to raise the level of resident preparation for future sessions.
